All incoming athletes are required to enter their pertinent medical information on our electronic medical records system, SportsWare. You will receive instructions from your coach on how to access that website. All physical forms must be returned to the Athletic Training department by July 15 each year . If you have any questions please contact Rachel Johnson at 320-308-2590 or rljohnson@stcloudstate.edu.
